The congenital duplication of the big toe implies the anatomical dislocation in the varus of the main big toe for two reasons: encumbrance given by the internal duplicate big toe, increased space of the first commissure, presence of a metatarsophalangeal joint which measures twice the width because it must accommodate two bony digital elements that articulate in common.

The surgical approach consists of the removal of the internal supernumerary big toe, osteotomy of bone separation, and the excess portion of the head of the first metatarsal. Also rerouting of excess extensor hallucis pro adductor. Articular osteosynthesis with steel wire between the metatarsal head and the base of the first phalanx, to obtain a longitudinal realignment of the first ray.

In addition, the reduction and modeling of the common nail is performed. Finally, the intermetatarsal ligament is reconstructed and the excess skin is modeled.
